Welcome to the 504 Plan Form Assistant
This interview will help you write a letter to your child's principal requesting the development of a 504 Plan.
If your child’s needs do not qualify for an IEP, they may be eligible for a 504 plan instead. Your child is eligible for a 504 plan if they have a mental or physical impairment that substantially limits learning or another life activity.
This interview will take about 10 minutes. At the end of the interview you can download or email a .pdf version of the letter with instructions for next steps.
